Choice DNA Employer Program
Dear Valued Client,
Thank you for your interest in setting up an account with Choice DNA’s drug testing program.
Employers who have at least 3 employees and intend to conduct drug testing are eligible to establish an employer account for drug and alcohol testing services. Individuals (donors) are not eligible to establish an EMPLOYER account and must pay for services when scheduled. Listed below you’ll find information regarding our testing services, fees and other Drug Free Work Place Program information.
Testing Center Locations
Choice DNA Drug Testing has testing centers and affiliated collection sites in all Cities Nationwide and in most cases our centers are within minutes of an employer’s business address or an employee’s home address. If your company has multiple locations in other Cities or States, Choice DNA can schedule and manage the testing program for all locations.
Employer Account Set Up
Establishing an account with Choice DNA Drug Testing is fast and easy. There is no initial set up fee required. Simply complete the electronic new account profile form below and establish the way in which you desire to be invoiced for testing services. We’ll email you a drug testing agreement to sign. Once we receive your agreement you can begin testing.
