Schedule An Appointment

Scheduling is easy. Once you complete the appointment form a representative will call you right away to confirm your appointment. They’ll let you know exactly what you need to bring to the appointment and how long your test results will take. They will also confirm the address of the location to have your DNA collected. 

*Please note that our DNA centers do NOT take walk-ins. An appointment is always required. Also know that after you fill out the appointment form to your left you will then need to talk to a representative to confirm your appointment
